Hi Stephanie, 

We did find that there was a bug in the cem routine that was causing these missing strata. It shouldn't affect the weights or any output, but it did cause problems with the strata. To install a beta version that doesn't have this problem, you can run this code in Stata:

ado uninstall cem
net from https://dl.dropboxusercontent.com/u/5379343/
net install cem, force
