dear all,
I am new to this mail list, and I hope I am answering in the right way.
In my understanding, the proper use of sample weights and counterfactual models ( PSM and
CEM) is not straightforward.
I am currently revising a old conference paper that propose "adjusted CEM
weights".
You may want to have a look to it.

Hi Stephanie,
We did find that there was a bug in the cem routine that was causing these
missing strata. It shouldn't affect the weights or any output, but it did
cause problems with the strata. To install a beta version that doesn't have
this problem, you can run this code in Stata:
ado uninstall cem
net from
https://dl.dropboxusercontent.com/u/5379343/
net install cem, force
Cheers,
Matt

Did you ever find a response to the question about
STATA Output from CEM-
matched observations with missing strata? We are finding the same problem
with our data. None of the matching variables have missing data, yet many
of the observations are being assigned to a ?missing? stratum (complete
with CEM weights for the missing stratum).
Any insights?
Thanks!
